Name change please

I left BTU and would like just Armalite as my nick if possible. I'm guessing that some day you guys will be playing Nova again and I want to be on here harrassing all you fatties when that day comes :)

You need to send in a form 302 signed by you and a parent..
To cover the cost of the name change a £30 cheque or postal order will also need to accompany form 302, this has to be filled out in BLACK pen and in block capitals. No crossings out or tipex will be accepted as this confuses the scaning machine (SOUP) that processs the forms.

To get form 302, form 9754 needs to be filled out and faxed or posted too us, Form 9754 can be found HERE (http://www.conveyancingsw.com.au/images/map.gif)
